TERMS & CONDITIONS SUMMARY – SUMMER FENCING CAMP

AGREEMENT TO TERMS
By signing below, you (or your minor child) agree to participate in Ohio Fencing Center (“OFC”) Summer Fencing Camp programs under the following terms.
1. ELIGIBILITY & REQUIREMENTS
☑ USA Fencing Membership Required – All campers must have a valid USA Fencing membership before the start of camp; learn more about USA Fencing Memberships here.
☑ Age Requirements – Minors under 18 require parent/guardian consent and signature.
☑ Medical Clearance – Campers must be physically capable of participating in athletic activity.

2. ATTENDANCE
☑ Beginner Camps – Campers must attend the first day of camp; no exceptions. Full-session registration is required (no daily/drop-in rates. Each session requires a minimum enrollment of at least 6 campers to run as scheduled and is limited to 12 to maintain small -group instruction.
☑ Competitive Camps – Requires a minimum enrollment of at least 6 campers to run as scheduled and is limited to 12 to maintain small -group instruction. A $125 day rate is available for those who cannot attend the full week.
3. PAYMENT, CANCELLATION & REFUNDS – SUMMER CAMP
☑ Payment Due in Advance – All camp fees must be paid in full before camp participation begins.
☑ Non-Refundable – Camp payments are non-refundable except for documented medical emergencies or OFC‑initiated cancellations.
☑ Camp Cancellations – Before May 1: Full refund via credit towards any OFC group glass/private lesson package. Between May 1-31: A $150 administrative fee applies. After May 31: No refund credit issued.
4. ASSUMPTION OF RISK & LIABILITY WAIVER
I acknowledge that fencing is a contact sport involving inherent risks, including but not limited to: bruises, sprains, strains, fractures, concussions and head injuries, serious injury or, in rare cases, death. I voluntarily assume all risks and agree to release, waive, discharge, and hold harmless Ohio Fencing Center, its owners, coaches, staff, and affiliates from any and all claims, damages, or injuries arising from participation in Summer Fencing Camp, including those caused by ordinary negligence of OFC. For Minors: As parent/guardian, I assume all risks on behalf of my child and agree to indemnify OFC for any claims arising from my child’s participation.
5. CONCUSSION AWARENESS (OHIO LAW)

☑ I have received and reviewed the Ohio Department of Health Concussion Information Sheet.

☑ I understand that any camper showing signs of concussion will be removed from activity immediately.
☑ Medical clearance from a physician is required before returning to camp after suspected concussion.
6. MEDICAL AUTHORIZATION
In the event of a medical emergency during camp, I authorize OFC staff to: seek emergency medical treatment for myself or my minor child, contact emergency medical services (EMS) if necessary, and share relevant medical information with medical professionals.
7. CONDUCT & DISMISSAL – CAMP EXPECTATIONS
☑ I agree that my child will follow all OFC safety protocols, facility rules, and USA Fencing regulations.
☑ I understand that disruptive behavior, unsafe conduct, or rule violations may result in dismissal from camp without refund.
☑ Respectful behavior toward coaches, staff, and fellow campers is required at all times.
8. EQUIPMENT
☑ My child will use all OFC camp equipment responsibly and return borrowed gear in good condition.
☑ I am responsible for damage to equipment caused by misuse or negligence.
☑ Any personal equipment brought to camp must meet USA Fencing safety standards.
9. PHOTO/VIDEO RELEASE
☑ I consent to OFC photographing or recording my (or my child’s) participation in Summer Fencing Camp for promotional, instructional, or archival purposes.
☑ Images may be used on OFC’s website, social media, and marketing materials.
10. INDEMNIFICATION
I agree to indemnify and hold harmless OFC from any claims, losses, damages, or expenses (including attorney fees) arising from my or my child’s participation in Summer Fencing Camp, breach of these Terms, violation of any law or third‑party rights, or any injury or damage caused by me or my child.
11. GOVERNING LAW
These Summer Fencing Camp Terms are governed by Ohio law. Any disputes must be resolved in Franklin County, Ohio courts.
12. ENTIRE AGREEMENT
This summary, combined with the full Terms & Conditions available at www.ohiofencingcenter.com/terms-and-conditions, constitutes the entire agreement between the camper/participant and OFC for Summer Fencing Camp.
PARTICIPANT ACKNOWLEDGMENT
I certify that:
✓ I have read and understand these Summer Fencing Camp Terms & Conditions.
✓ I voluntarily agree to all terms, including assumption of risk and liability waiver.
✓ All information provided is accurate and complete.
✓ I am legally authorized to sign on behalf of the minor participant (if applicable).
